Below is a link to the forms available for the School of Business Administration.

In order to use the form you must download it on your computer, save the changes and e-mail the final copy to advisingSBA@notes.udayton.edu.

Change Cluster

This form allows you to declare or change your thematic cluster.  For more information on clusters click here.

Change Major or Minor

This form allows business students to change their major or minor in the School of Business.  You must already be in the School of Business to change majors.  After the form is completed, a faculty advisor will be assigned to you from the department offering your major.

Drop / Add Form

This form allows students to add or drop a course or change grading options.  Please check registrar.udayton.edu for deadlines.  If you need a signature, stop by the Advising Center, MH 108.

FERPA - Family Educational Rights and Privacy Act

FERPA gives parents certain rights with respect to their children's educational records. These rights transfer to the student when he or she reaches the age of 18 or attends a school beyond the high school level. Please print, sign and date this form and return to the Advising Center (MH 108). This form will remain on file for one academic school year.

The MBA-Ready Program is a selective program for a limited number of admitted, qualified UD engineering and sport management students.  Apply online.
