Address Change Directions

We want to help you fill out the address change form correctly.  Below, you will find definitions and explanations for the basic components of our form. 

Date of Birth:  Month / Day / Year

Permanent Address: 

  • International or exchange students, the permanent address refers to your address in your home country outside of the United States.  
  • Domestic Undergraduate or other dependent students, the permanent address refers to your parent/guardian’s address.  
  • Domestic Graduate, Law or independent students, the permanent address refers to your current place of residence. 

Local Address while attending UD: 

  • International or exchange students, the local address refers to your address while attending the University of Dayton in the United States (on campus or off campus).  
  • Domestic Undergraduate or other dependent students, the local address refers to your address on campus or landlord housing.  
  • Domestic Graduate, Law or independent students, the local address refers to your local address if different from your permanent address.  

Billing Address:  Please use only if you would like your bill sent to an address other than your Permanent Address or Local Address.   

Where should we send your BILLS?:  Please tell us if you would like your bills to be sent to your Permanent Address, Local Address or Billing Address.  We will only send your bill to one address.  

Where should we send your MAIL?:  Please tell us if you would like your mail from the University of Dayton to be sent to your Permanent Address, Local Address or Billing Address.  We will only send your mail to one address.
