Counselor Recommendation Form

In order to have a complete admission application, students must submit a counselor recommendation form. As a guidance counselor, you should complete the counselor recommendation form (PDF) and mail it to the University of Dayton with:

  • An official transcript of courses taken and grades received.
  • Results of either the ACT or SAT.
  • The current senior-year courses in which the student is enrolled and grades for any courses completed.
