Q.  Are there any medical forms that I need to fill out prior to coming to U.D.?

A.  All students are required to have 2 Measles, Mumps, and Rubella vaccines (MMR).  The immunization requirements must be submitted on-line during the registration process for undergraduate students prior to coming to the University.  Graduate, law, and international students will need to submit the immunization card provided in the enrollment packet.

Q.  Where is the student Health Center located?

A.  The Health Center is located in the lower level of Gosiger Hall.

Q.  Do I need to make an appointment for services at the Health Center?

A.  Yes.  Appointments can be made by calling 937-229-3131.  Please give the receptionist a brief description of what you need to be seen for so you will be scheduled in the appropriate time slot.  You will need to arrive 15 to 20 minutes prior to your appointment time.  Walk-ins are for emergencies only.

Q.  Who will provide services for patients (students) at the Student Health Center?

A.  The Health Center is staffed by 2 full time board certified Family Practice Physicians, registered nurses, a receptionist, and office administrator.

Q.  How do I pay for services at the Student Health Center?

A.  There is no charge for consultations with the nurse or physician for undergraduates and law students.  Graduate students are charged for each Heath Center visit because they do not pay a Basic University Fee.

All charges incurred are placed on the student account at the Bursars Office.  Insurance companies are not billed directly; however, students may request an itemized bill.  Local billing directly to the student may be requested in certain situations.

Q.  Can my parents receive information about my visit?

A.  No. Health Center visits are strictly confidential.  Exceptions are made if a clear and imminent danger exists, or with the consent of students 18 years of age and older on a per visit basis.

Q.  Do you write medical excuses for missed classes?

A.  No.  If the professor requests confirmation of illness, the professor may submit a request on the Student Health Center website.  Only verification dates will be given.

Q.  Can I receive allergy injections at the Student Health Center?

A.  Yes.  A private allergist must provide serum and a signed treatment plan.  The initial  allergy injections  must be given at the allergist's office.  We ask that you bring your antigen and instructions to the Student Health Center and we will store them properly for you.  An appointment is necessary and a physician must be present.

Q.  Can I have my prescription filled at the Student Health Center?

A.  The Student Health Center does not have an outpatient pharmacy.  However, for you convenience, we do carry some commonly prescribed medications and make them available to students if ordered by our physician.  If the Health Center does not carry the medication, or if the student wants to use their insurance, we can order this from CVS  Pharmacy and the medication will be delivered to the Health Center.

    For prescriptions written by outside doctors, the student may drop the prescription off at the Health Center and it will be sent to CVS Pharmacy along with a  copy of your insurance card.  Please allow 24 to 48 hours for delivery of medication to the Health Center.  The charge for medications will be placed on the student's bursar account.

    You may also take the prescription to any local pharmacy and have it filled there.  Charges will need to be paid at the time of the transaction and will not be applied to the Bursar account.

Q.  What do I do after hours and on weekends if I am sick?

A.  If your are ill or injured, call U.D. Public Safety, 937-229-2121.  They will transport you to a local hospital for treatment.  There are also local Urgent Cares for students with their own transportation.
