In order to use the form you must download it on your computer, save the changes and e-mail the final copy to

Change Major or Minor

This form allows business students to change their major or minor in the School of Business.  You must already be in the School of Business to change majors.  After the form is completed, a faculty advisor will be assigned to you from the department offering your major.

Drop / Add Form

This form allows students to add or drop a course or change grading options.  Please check for deadlines and signatures needed. You may obtain this form and further instructions in the Advising Office, MH 108.

FERPA - Family Educational Rights and Privacy Act

FERPA gives students certain rights with respect to their educational records. Please print, sign and date this form and return to the Advising Center (MH 108) if you wish to allow your parents or guardians access to your educational records.

Transfer Credit Policy and Approval Form

Please review our Transfer Credit Policy and complete the Transfer Credit Approval Form before registering for classes at another college or university.  Please allow a minimum of two weeks for this form to be processed.
