Contractor Approval Process

The University of Dayton has developed a process to approve contractors hired to work for the University.  This two-step process will help the University to control risks by hiring reputable and fully insured contractors. Step 1: Contractor Health and Safety Requirements

The University of Dayton has developed Health and Safety Requirements for all contractors hired by the University. These requirements are captured in two documents: (1) Contractor Health and Safety Questionnaire and (2) Contractor Management Health and Safety Requirements. Complete these two documents and submit them to your University of Dayton representative.  

These documents contain performance expectations applicable to all contractors and their personnel regarding health and safety responsibilities while working at the University of Dayton. To ensure the protection of the University’s students, faculty, staff, visitors, and property, contractors are expected to provide a safe and secure workplace and operate in an environmentally sound manner.

Step 2: Terms and Conditions

The University of Dayton has developed standard Terms and Conditions that are used when contracting for various types of design, construction, and related services.  Contractors should print and sign the Terms and Conditions and submit the document to their University of Dayton representative.  The University representative should also sign this document and submit it to Environmental Health & Safety/Risk Management.

The Terms and Conditions require that Contractors submit a current Certificate of Insurance (COI) for the work being performed. To assist with preparing the COI per UD’s specifications, please see the: (1) Minimum Insurance Requirements (narrative) and the (2) Vendor Insurance Requirements (chart) listed in the Related Links below. Contractors should review both the narrative and the chart and select one of the insurance requirements formats to follow to prepare the certificate. The University of Dayton should be listed as the Certificate Holder on each certificate. A sample COI is also included in the Related Links to serve as a guide in preparing the COI. Submit your COI to your University representative.

Other Related Documents that Contractors Need to Submit

Contractors must also complete the Non-Employee Forms and follow the Key Distribution Process. (1) Contractors are required to submit the Non-Employee Forms and give the forms to their University representative. Human Resources will use these forms to document contractors who require physical access to campus facilities. (2) Contractors who require access to campus facilities are also required to follow the Key Distribution Process. This process requires the Contractor to prepare a letter describing their request. The Key Distribution Process link will take contractors to a sample letter that should be customized and submitted to their University representative.
