Professors may complete this form to receive confirmation of illness for a particular student. Only verification dates will be given.
Medical Excuse Policy
The University of Dayton Student Health Center does not issue written excuses for illness or injury.
Students are responsible for notifying professors of absence from class due to short-term illness or injury. The student health center will verify on request that a student has been evaluated or seen and treated in the health center. The professor may request this through the student visit verification form.
Confidentiality is maintained and a reason for the visit and/or diagnosis will not be given without written permission from the student.
THE STUDENT IS RESPONSIBLE FOR CONTACTING EACH PROFESSOR TO DISCUSS MISSED CLASSWORK.
