Injections ordered by outside physicians

Injectable medication orders from an outside physician are reviewed by the Medical Director to determine if they may be administered at the Health Center.  If approved, orders must include the following information and submitted each school year:

  • Patient's name and date of birth
  • Medication name, dosage, frequency of injection and the date of the last injection
  • Reason for the injection and diagnosis code

In the case of DEPO-PROVERA, we can only administer this for a non-contraceptive condition.

If receiving certain immune suppressant medications, for example, Enbrel, Humira, Simponi, or Methotrexate, it is recommended that you have annual tuberculosis testing.  The Health Center can provide this testing for you.  Please call 937-229-3131 for any questions or to schedule an appointment.
