Assessment Forms and Templates

Each year units throughout the university submit to the University Assessment Committee (UAC) summaries of actions taken as a result of assessment. The units keep in their files complete reports of their assessment efforts, including results of measurements used. Only a brief summary is submitted to the UAC.

Units have the opportunity to submit requests to the UAC for extra funds to conduct assessments. Typically these are one-time requests for special projects not covered in their departmental budgets.
